Included. The live sports Amazon carries come with the Prime membership through Prime Video, and there is no sports pass to add. The things that cost extra are the things that cost extra for every viewer, which are rentals, purchases, and add on channels. The confusion is not the sports. It is the other line items sitting beside them.

What is included
Live events Amazon carries, plus the on demand catalog that comes with the membership, all included. There is no sports surcharge and no separate sports login. If the membership is active, the included tier is active with it.
That is the same deal a member gets on the rest of the digital bundle, where the entry tier is included and the fuller versions are separate purchases.
What carries a price of its own
Rentals, digital purchases, and add on channels are billed on top of the membership, and they are billed the same way whether or not a game is involved. A live event Amazon carries is not one of those. A film you rent to watch while waiting for kickoff is.
Worth naming plainly. The add on button is always close to the play button, and that proximity is the whole reason people think sports cost extra. The membership covers what Amazon includes. It does not cover what you choose to buy.
How to tell which is which
Check whether the title is labelled as included with the membership or as a rental or purchase before you start it. Included titles play on the membership alone. Anything showing a price is a separate transaction, and none of them change what the membership already grants you.
If a game you expected to watch is not on the included list, the honest reason is that Amazon is not carrying it, not that a hidden tier was withheld. Rights decide the slate. Read the slate, not the rumour.
Buy it if the included slate covers your games and you can ignore the add on buttons.
Skip it if the games you want sit behind a separate purchase. Included and available are not the same word.
